Medication Regimen Review 

Medication regimen is reviewed for:

  • Drug interactions

  • Side effects (reducing confusion, falls, and other common geriatric problems)

  • Appropriate use & under treatment

  • Medication outcomes

  • Pain control

  • Laboratory reviews

  • Audit of transcription or new medication

Medication Administration Record

Good Day Consultant Pharmacist will evaluate:

  • Transcriptions for accuracy and freedom of general errors

  • Accuracy of times of administration

  • Interactions with other medications that are given at the same time

  • Accuracy and completeness of charting procedures

  • Accuracy of PRN use of medications with proper documentation

  • Accuracy of documentation for discontinued or short term medications

  • Procedures for physician notification of refused or unavailable medication

Medication Storage Area Inspection

Each area is inspected to ensure that:

  • Medications are not expired

  • Proper labeling and storage

  • A narcotic count procedure is completed

  • Audits are documented and completed on PRN and psychotropic medication

Medication Pass Observation

This is a report provided with each QMAP’s evaluation.

A detailed report monitoring QMAP med passes will include:

  • The overall review

  • Identification of QMAP errors along with corrections

  • Review of what to do in common situations

Regulatory Compliance

Providing assistance in a variety of areas, including:

  • Identification of medication issues and trends

  • Pre-survey chart reviews and audits

  • Survey preparation and support; participation on-site during survey

  • Development and execution of pre and post-survey action plans

Staff Education

Can be customized covering a range of topics, including:

  • New medications

  • Med pass- What every QMAP should know about med passing and more

  • Fall risk with certain medications

  • Balance and medications

  • Herbs and supplements

  • Charting and ordering

  • Pain management for the elderly
