for the patient. Sometimes the testing can be done

through the practitioner’s office and sometimes the

best testing may be either via saliva and/or blood

spot testing with kits Lori has available. Lori will try

to save the patient money this way by helping the

patient choose the best testing options for their

particular situation.

These appointments require the patient to fill out a

history form and bring in any recent testing results

and information on medications and/or hormone

therapy the patient is currently taking. The

consultation packet (History Form) can be accessed

on our website www.gooddaypharmacy.com or

picked up at one of our local Good Day Pharmacies.

Please write at the top of the history form that you
